CONDITIONS OF PARTICIPATION

The company ALPHA - H.R., s.r.o., operator of the ADELI Medical Center with its registered office in Ducové 180, 922 21 Ducové, company number: 52 230 236, tax number: 2120967992, registered in the Commercial Register of the Regional Court Trnava, Section Sro, Insert No. 43863/T, is the organizer of the prize draw for two prizes "5 trainings on the ICAROS devices". The aim of the prize draw is to raise awareness of the promotion of health and the urgency of rehabilitative treatments, as well as to support and promote the services of the ADELI Medical Center.

REGISTRATION

The registration of the participants of the raffle will only take place on October 28, 2023 from 10:00 to 17:00 at the venue: EUROVEA Bratislava, presentation of the ADELI Medical Center. Each adult person with their own e-mail address can register once.

CONDITIONS FOR A VALID REGISTRATION

Registration of a participant of the raffle includes filling in a contact form on www.mysmeadeli.com and uploading a photo to the form, which was obviously taken at the event in EUROVEA on 28.10.2023. At the same time, each registered participant gives his consent to the publication of his uploaded photo within the framework of the provisions stated in the PERSONAL DATA.

The participant of the raffle must be at least 18 years old and must not be an employee or external employee of the company APLHA H.R. s.r.o.. The confirmation of registration for the participant of the raffle is the e-mail that the registered participants will receive to the e-mail address they specified in the contact form, which must be functional.

RAFFLE RULES

All validly registered e-mail addresses take part in the raffle. These are sorted by number and each e-mail address is assigned a lot number in a clear table. The draw takes place by physically drawing the numbers. Each number represents one e-mail address in the list of registered addresses. The highest number represents the number of valid registered e-mail addresses.

The date for the draw is in the week after 28.10.2023, at the latest on 3.11.2023. The draw will take place at 15:00 at ADELI Medical Center and will be held in public.

The winners of the two prizes "5 trainings on the ICAROS devices" will choose one person who will redeem the prize at ADELI Medical Center if there are reasons why the winner cannot attend in person. We will ask the winner in advance for permission to publish their picture.

The independence and fairness of the raffle is guaranteed by on-site checks and by the verification of the e-mail lists by a marketing agency. The draw may not be influenced by any external factors or persons who may have an interest in the outcome. The transparency of the raffle is guaranteed by the public.

REWARD

We are giving away 2 prizes in the prize draw: 5 training sessions on ICAROS equipment at ADELI Medical Center in Piešťany in the presence of a trained physiotherapist. One training session lasts 30 minutes. Depending on availability at ADELI Medical Center , several training sessions can be carried out in succession. Please consider your state of health and consult a doctor before training if you have any doubts. In case of non-realization of the prize for any reason, the prize can be transferred to another person.

PERSONAL DATA

By registering, each participant of the raffle gives his consent to the use and storage of his personal data to the following extent:

- First name, last name and email address + photo uploaded on www.mysmeadeli.sk,

- for the purpose of carrying out the competition (e.g. conducting the competition, handing over and receiving the prize) for the duration of the draw and for a further 3 years thereafter. The consent is voluntary and can be revoked at any time in writing to the address of the responsible person.

Pursuant to Sections 19-30 of Act No. 18/2018 Coll. on the Protection of Personal Data, as amended, the Participants have, in particular, the right to information on the status of processing of their personal data, the right to rectify incorrect or outdated personal data during processing, the right to erasure of personal data when the purpose of their processing has been fulfilled, and the right to object to the use of their personal data for marketing purposes by submitting a free written request. Withdrawal of consent to one of the aforementioned subjects does not affect the duration of the consent given to the other.

The participant of the raffle acknowledges and agrees that the organizer or intermediary, within the terms agreed with the organizer, are entitled to collect, process (within the meaning of the Personal Data Protection Act) and use the data using automated, partially automated or other than automated processing methods. By participating in the raffle, the participant gives his/her consent to the organizer in accordance with § 12 of the Civil Code:

- for the creation and use of his image, written statements, image recordings and image and sound recordings concerning his person or personal statements taken by the organizer on 28.10.2023 in Piešťany.

- to use photo and audio-video recordings in connection with the raffle and the handing over of prizes (hereinafter referred to as "recordings") for commercial and exhibition purposes in all communication media, regardless of their nature and purpose, in the usual manner, with subsequent editing and, if necessary, combination with other works or inclusion in a collective work.

The participant of the raffle grants this consent to the organizer without factual, temporal, quantitative or spatial restrictions and may revoke it at any time. This consent also extends to third parties to whom the organizer makes the recordings available in accordance with their designation.

TAXES

The Organizer is not responsible for any taxes arising from the prize pursuant to the relevant Act No. 595/2003 Coll. on income tax, as amended. The winner has no legal right to the exchange of the prize or payment of its value in the form of money or other compensation. The participant of the raffle acknowledges that winnings from bets and games cannot be claimed in court in accordance with the applicable legal provisions.

SPECIAL PROVISIONS

The Organizer reserves the right to change the rules of the raffle, the raffle itself, the duration and conditions of the raffle, including but not limited to the raffle rules, the date of the raffle, the number of winners and the types of prizes that are the subject of the raffle. The Promoter will provide reasonable notice of any changes to the Sweepstakes Rules and Conditions. The Organizer shall not be liable for any damage caused by incorrect information provided by the participants of the raffle in connection with not claiming or not using the prize. The winner has no legal right to the exchange of the prize/win or the payment of its value in the form of money or other compensation (the exchange of the prize for money also includes the exchange of the prize for securities, real estate, deposits in a deposit account or insurance, etc.).

The participant acknowledges that the prize cannot be claimed in court. In the event of a dispute in connection with the prize draw, the decision of the organizer is final and binding. The provisions of Sections 612 to 627 of the Civil Code, as amended, shall not apply to the prize. The participants of the raffle can view these rules on the website www.mysmeadeli.com and www.adelicenter.com.

By participating in the raffle, raffle participants agree to abide by these rules.
