TERMS AND RULES OF THE COMPETITION

The company ALPHA – H.R., s.r.o., operator of the ADELI Medical Center based in Ducové 180, 922 21 Ducové, ID: 52 230 236, VAT ID: 2120967992, registered in the Commercial Register of the District Court Trnava, Section Sro, File No. 43863/T, is the organizer of the competition and the raffle of a 500-euro voucher that can be redeemed during a REHABILITATION STAY AT ADELI, hereinafter referred to as the "Raffle". The goal of the raffle is the support and promotion of the services of the ADELI Medical Center. REGISTRATION The registration of participants in the raffle takes place exclusively from July 12 to 25, 2024, on the subpage ….. Every person with their own email address can register, however, each person can only participate in the competition with one self-created dish/recipe.
CONDITIONS FOR VALID REGISTRATION The registration of a participant in the raffle involves filling out a contact form on www.adelicenter.com and uploading a photo, the name, the recipe, and the preparation steps of a dish that was obviously created for the ADELICIOUS 2 competition. At the same time, every registered participant gives consent to the publication of their uploaded photo to the extent mentioned in the PERSONAL DATA section. The participant in the raffle must not be an employee or external collaborator of the company ALPHA – H.R. s.r.o.
Confirmation of registration will be sent by email to the email address provided in the contact form, which must be functional. RULES OF THE RAFFLE All validly registered email addresses that meet the conditions will participate in the raffle:

• A photo of the dish containing the 2 prescribed ingredients: Quark and apricots
• The name of the dish, the recipe and the preparation steps (please be brief) The draw will be made by drawing numbers.

Each number represents an email address in the list of registered addresses. The date of the draw is Saturday, July 27, 2024. The draw will take place at the ADELI Medical Center the afternoon and will be conducted as part of the ADELICIOUS program in the presence of customers, colleagues, and the public.
If the winner of the €500 voucher does not redeem the voucher themselves during their stay, they may designate a person who will use the voucher at ADELI. The winner is asked to give their consent in advance for the publication of the photo of the award ceremony and the patient redeeming the voucher.

The independence and fairness of the prize draw is guaranteed by on-site checks and verification of the e-mail lists by a cooperating marketing agency. The prize draw must not be influenced by external factors or persons who may have an interest in the result. The transparency of the prize draw is ensured by its openness to the public.

REWARD The prize in the raffle is a €500 voucher for services at the ADELI Medical Center Piešťany. The date will be agreed between the patient's family and representatives of the raffle organizer. A prerequisite for the start is a referral from a doctor at ADELI. If the stay does not take place for any reason, the prize can be transferred to another person. PERSONAL DATA By registering, each participant in the raffle gives their consent for their personal data to be used to the extent of:

may be appropriately used and stored in order to carry out the competition (e.g., conducting the competition, prize award and delivery), for the duration of the raffle and also afterwards for a period of 2 years. Consent is given voluntarily and can be revoked at any time in writing to the address of the controller. The participant has the rights according to §§ 19 – 30 of Act No. 18/2018 Coll. on the Protection of Personal Data, as amended, in particular the right to information about the status of the processing of their personal data that are the subject of processing, the right to correct incorrect or outdated personal data during processing, the right to erasure of personal data when the purpose of their processing has been fulfilled, the right to free written objections against the use of their personal data for marketing purposes. The revocation of consent regarding one of the mentioned subjects does not affect the validity of the consent given to the other subject. The participant in the raffle acknowledges and agrees that the organizer or intermediary, to the extent and under the conditions agreed with the organizer, is authorized to collect, process, and use personal data according to the Act on the Protection of Personal Data by means of automated, partially automated, or other than automated means. By participating in the raffle, in accordance with § 12 of the Civil Code, the participant grants the organizer of the raffle consent:

• to the creation and use of his image, his written statements, his image recordings and his image and sound recordings made of him or his personal statements by the organizer.
• Consent to the use of photo and audio-video recordings in connection with the raffle and the prize-giving ceremony (hereinafter "Recording") for commercial and demonstrative purposes in all communication media regardless of their type and purpose in all common ways, including their subsequent editing and their possible combination with other works or inclusion in a collective work.

The raffle participant grants the organizer this consent without any material, temporal, quantitative or spatial limitation and may revoke it at any time. This consent also extends to third parties to whom the organizer makes the recording available in accordance with its purpose. TAXES The Organizer shall not be liable for any taxes arising from the prize in accordance with the relevant Act No. 595/2003 Coll. on Income Tax, as amended. The winner is not legally entitled to the exchange of the prize or the payment of its value in the form of monetary or other compensation. The participant in the prize draw acknowledges that winnings from bets and games cannot be claimed in court in accordance with the applicable legal provisions. SPECIAL PROVISIONS The Organizer reserves the right to change the rules of the prize draw, the duration and the conditions of the prize draw, in particular, but not exclusively, the rules of the prize draw, the date of the prize draw, the number of winners and the types of prizes that will be drawn. The Organizer will publish changes to the rules and conditions of the prize draw in an appropriate manner. The organizer shall not be liable for any damage caused by incorrect information provided by the participants in connection with the failure to claim or use the prize. The winner has no legal right to the exchange of the prize/prize or the payment of its value in the form of monetary or other compensation (the exchange of the prize for money also means the exchange of the prize for securities, real estate, a deposit in a savings account or insurance, etc.). The participant of the game acknowledges that the prize cannot be claimed in court. In the event of a dispute regarding the prize draw, the Organizer's decision shall be final and binding. The provisions of Sections 612 to 627 of the German Civil Code, as amended, shall not apply to the prize.
