CONDITIONS AND RULES OF THE COMPETITION

The company ALPHA - H.R., s.r.o., operator of ADELI Medical Center with its registered office at Ducové 180, 922 21 Ducové, IČO: 52 230 236, DIČ: 2120967992, registered in the Commercial Register of the Regional Court Trnava, Section Sro, Insert No. 43863/T, is the organizer of the competition and the prize draw for a 500-euro voucher that can be redeemed during a REHABILITATION STAY AT ADELI, hereinafter referred to as the "Prize Draw". The aim of the prize draw is to support and promote the services of ADELI Medical Center. REGISTRATION Participants in the prize draw can only register from July 12 to 25, 2024 on the website ..... Anyone with their own e-mail address can register, but each person can only take part in the competition with a dish/recipe they have created themselves.
CONDITIONS FOR VALID REGISTRATION The registration of a participant in the prize draw involves filling in a contact form on www.adelicenter.com and uploading a photo, name, recipe and preparation steps of a dish that has obviously been created for the competition ADELICIOUS 2. At the same time, each registered participant consents to the publication of his/her uploaded photo to the extent mentioned in the PERSONAL DATA section. The participant of the prize draw may not be an employee or external collaborator of ALPHA - H.R. s.r.o.
Confirmation of registration will be sent by e-mail to the e-mail address provided in the contact form, which must be functional. RULES OF THE DRAW All validly registered e-mail addresses that meet the conditions will take part in the draw:

• A photo of the dish containing the 2 prescribed ingredients: Quark and apricots
• The name of the dish, the recipe and the preparation steps (please be brief) The draw will be made by drawing numbers.

Each number represents an e-mail address in the list of registered addresses. The date of the prize draw is Saturday, 27 July 2024. The prize draw will take place at ADELI Medical Center in the afternoon and will be held as part of the ADELICIOUS 2 program in the presence of customers, colleagues and the public.
The winner of the €500 voucher may, if they do not redeem the voucher themselves during their stay, designate a person who will use the voucher at ADELI. The winner will be asked to give their consent in advance to the publication of the image of the award ceremony and the patient who redeems the voucher.

The independence and fairness of the prize draw is guaranteed by on-site checks and verification of the e-mail lists by a cooperating marketing agency. The prize draw must not be influenced by external factors or persons who may have an interest in the result. The transparency of the prize draw is ensured by its openness to the public.

REWARD The prize of the draw is a 500 euro voucher for services at ADELI Medical Center in Piešťany. The appointment will be arranged between the patient's family and the representatives of the raffle organizer. The prerequisite for the start is an indication by a doctor at ADELI. If the stay is not carried out for any reason, the prize can be transferred to another person. PERSONAL DATA By registering, each participant in the prize draw consents to their personal data being processed to the extent necessary for the purposes of the prize draw:

may be used and stored appropriately in order to carry out the competition (e.g. holding the competition, awarding and presenting prizes) for the duration of the prize draw and for a period of 2 years thereafter. Consent is given voluntarily and can be revoked at any time in writing to the administrator's address. The participant has the rights under Sections 19 - 30 of Act No. 18/2018 Coll. on the Protection of Personal Data, as amended, in particular the right to information on the status of the processing of their personal data subject to processing, the right to rectification of incorrect or outdated personal data during processing, the right to erasure of personal data when the purpose of their processing has been fulfilled, the right to free written objection to the use of their personal data for marketing purposes. Withdrawal of consent from one of the above subjects does not affect the validity of the consent given to the other subject. The participant of the prize draw acknowledges and agrees that the organizer or intermediary is entitled to collect, process and use personal data by automated, partially automated or other than automated means in accordance with the Personal Data Protection Act to the extent and under the conditions agreed with the organizer. By participating in the prize draw, the participant grants consent to the organizer of the prize draw in accordance with Section 12 of the German Civil Code:

• to the creation and use of his image, his written statements, his image recordings and his image and sound recordings made of him or his personal statements by the organizer.
• Consent to the use of photo and audio-video recordings in connection with the prize draw and the award ceremony (hereinafter "recording") for commercial and verifiable purposes in all communication media, regardless of their nature and purpose, in all usual ways, with their subsequent editing and their possible combination with other works or inclusion in a collective work.

The raffle participant grants the organizer this consent without any material, temporal, quantitative or spatial limitation and may revoke it at any time. This consent also extends to third parties to whom the organizer makes the recording available in accordance with its purpose. TAXES The Organizer shall not be liable for any taxes arising from the prize in accordance with the relevant Act No. 595/2003 Coll. on Income Tax, as amended. The winner is not legally entitled to the exchange of the prize or the payment of its value in the form of monetary or other compensation. The participant in the prize draw acknowledges that winnings from bets and games cannot be claimed in court in accordance with the applicable legal provisions. SPECIAL PROVISIONS The Organizer reserves the right to change the rules of the prize draw, the duration and the conditions of the prize draw, in particular, but not exclusively, the rules of the prize draw, the date of the prize draw, the number of winners and the types of prizes that will be drawn. The Organizer will publish changes to the rules and conditions of the prize draw in an appropriate manner. The organizer shall not be liable for any damage caused by incorrect information provided by the participants in connection with the failure to claim or use the prize. The winner has no legal right to the exchange of the prize/prize or the payment of its value in the form of monetary or other compensation (the exchange of the prize for money also means the exchange of the prize for securities, real estate, a deposit in a savings account or insurance, etc.). The participant of the game acknowledges that the prize cannot be claimed in court. In the event of a dispute regarding the prize draw, the Organizer's decision shall be final and binding. The provisions of Sections 612 to 627 of the German Civil Code, as amended, shall not apply to the prize.
