CONDITIONS OF PARTICIPATION

The company ALPHA – H.R., s.r.o., operator of the ADELI Medical Center with its registered office at Ducové 180, 922 21 Ducové, IČO: 52 230 236, DIČ: 2120967992, entered in the Commercial Register of the District Court of Trnava, Section Sro, File No. 43863/T, is the organizer of the RAFFLE for a 500 Euro voucher, which can be redeemed during a REHABILITATION STAY AT ADELI, hereinafter referred to as the “Raffle”. The purpose of the raffle is to support and promote the services of the ADELI Medical Center.

REGISTRATION

Registration for participants in the prize draw Bratislava exclusively on June 6, 2024, between 3:00 p.m. and 6:00 p.m. at ADELI Medical Center at the Karlovarský Festival Zdravia event in Bratislava . Anyone with their own email address can register.

CONDITIONS FOR A VALID REGISTRATION

Registration for the raffle involves filling out the contact form on www.adelicenter.com and uploading a photo to the form that was clearly taken by the participant at the ADELI BABY ZONE event at the aforementioned time. At the same time, every registered participant agrees that their uploaded photo may be published to the extent specified in the PERSONAL DATA section. The raffle participant must not be an employee or an external collaborator of the company APLHA – H.R. s.r.o. Confirmation of registration is provided via email, which registered participants will receive at the email address provided in the contact form, which must be functional.

RAFFLE RULES

All validly registered e-mail addresses will be entered into the prize draw. These will be sorted in numerical order. The draw is carried out physically by drawing numbers. Each number represents an e-mail address in the list of registered addresses. The highest number represents the number of validly registered e-mail addresses.

The date for the raffle is Saturday, June 6, 2024. The raffle will ADELI Medical Center in the afternoon at the ADELI Medical Center and will be conducted in the presence of customers, colleagues, and the public.

The winner of the voucher worth 500 euros will independently choose the person who will redeem the voucher at the ADELI if they do not redeem the voucher themselves during their stay. We ask the winner to agree in advance to the publication of a photo/picture of the winner and the patient who will redeem the voucher.

The independence and fairness of the prize draw is guaranteed by an on-site check and a check of the e-mail lists by a cooperating marketing agency. The prize draw must not be influenced by external factors or persons who could have an interest in the outcome. The transparency of the prize draw is guaranteed by its openness to the public.

PRICE

The prize in the raffle is a €500 voucher for services at ADELI Medical Center Piešťany. The date will be agreed between the patient's family and representatives of the raffle organizer. A doctor's referral from ADELI is required before treatment can begin. If the stay does not take place for any reason, the prize can be transferred to another person.

PERSONAL DATA

By registering, each participant in the prize draw agrees that their personal data may be processed to the extent of:

may be used and stored in an appropriate manner for the purpose of running the competition (e.g. running the competition, handing over and receiving the prize) for the duration of the competition and for a period of 3 years thereafter. Consent is given voluntarily and can be revoked at any time in writing to the address of the controller's registered office.

The participant in the prize draw has the rights pursuant to § 19 – 30 No. 18/2018 Coll. on the protection of personal data as amended, i.e., in particular the right to information about the status of the processing of their personal data that is the subject of processing, the right to correct incorrect or outdated personal data during processing, the right to deletion of personal data if the purpose of its processing has been fulfilled, the right to object to the use of their personal data for marketing purposes on the basis of a free written application. The withdrawal of consent from one of the mentioned entities does not affect the duration of the consent granted to the other.

The participant of the prize draw acknowledges and agrees that the organizer or intermediary are entitled to collect, process (within the meaning of the Personal Data Protection Act) and use the data in accordance with the legal order of the Slovak Republic within the scope and under the conditions agreed with the organizer. By participating in the prize draw, the participant grants consent to the organizer of the prize draw in accordance with Section 12 of the Civil Code:

• to make and use his image, written statements, images and image and sound recordings relating to his person or personal statements made by the organizer at the Karloveský Festival Zdravia on 6 June 2024 in Bratislava .

• for the use of photo and audio-video recordings in connection with the raffle and the handing over of the main prize (hereinafter “Image”) for commercial and evidentiary purposes in all communication media, regardless of their nature and intended purpose, in all customary ways, with their subsequent processing and possible connection with other works or inclusion in a collective work.

The participant in the prize draw grants the organizer this consent without restriction as to subject matter, time, quantity or territory and may revoke it at any time. This consent also applies to third parties to whom the organizer makes the image available in accordance with its purpose.

TAXES

The organizer is not liable for taxes arising from the winnings in accordance with the relevant Act No. 595/2003 Coll. on Income Tax, as amended.

The winner has no legal claim to the exchange of the prize or payment of its value in the form of monetary or other compensation. The participant in the prize draw acknowledges that winnings from bets and games cannot be claimed in court under applicable law.

SPECIAL PROVISIONS

The Organizer reserves the right to change the rules of the draw, to cancel the draw, the duration and conditions of the draw, in particular, but not exclusively, the rules of the draw, the date of the draw, the number of winners, as well as the type of prizes that will be the subject of the draw. The Organizer will publish the changes to the draw rules in an appropriate manner. The Organizer shall not be liable for any damages arising in connection with incorrect data provided by the participants of the prize draw in connection with non-registration or failure to use the prize. The winner has no legal claim to the exchange of the prize/winnings or to the payment of its value in the form of monetary or other compensation (the exchange of the prize for money also means the exchange of the prize for a security, a property, a deposit in a savings book or an insurance policy, etc.).

The player acknowledges that the prize cannot be claimed in court. In the event of disputes regarding the draw, the Organizer's decision is final and binding. The provisions of Sections 612 to 627 of the German Civil Code, as amended, shall not apply to the prize. By participating in the prize draw, prize draw participants agree to comply with these rules.
